Bridgeforth Consulting Group was founded by a social worker with professional and lived experience navigating complex care and support systems, to bridge the gap between individuals, families, and the systems they rely on for care, benefits, and coordination.
As services become increasingly fragmented, families are often left to manage administrative requirements, documentation, and coordination across multiple programs with limited guidance. At the same time, organizations and managed care partners face challenges maintaining continuity, follow-through, and engagement beyond clinical settings.
Bridgeforth works at the intersection of care coordination, administrative support, and organizational systems. We help individuals and families understand their options, complete critical documentation, and move forward with clarity, while also supporting agencies and partners seeking stronger coordination, structure, and reliability across services.
